Patient Forms
At Endoscopy Center of Delaware, your time is as valuable to us as it is to you. That’s why we provide online access to a variety of forms that patients may complete before their visit. The following documents will help us expedite the check-in process for your appointment. Please take a moment to download and print the appropriate form(s).
Insurance and Billing
Endoscopy Center of Delaware accepts most major insurance. We will bill your insurance carrier for you; however, we cannot tell you what your policy will cover. Since your insurance policy and coverage are between you and your carrier, we urge you to contact them prior to your procedure. You will be responsible for any co-pays and deductibles.
Express Scheduling
If you’re over 50 and haven’t had a colonoscopy, take advantage of the Express Scheduling Option offered through our affiliated practices. By scheduling directly with the physician practices, you see a Board Certified specialist at our center and have your procedure on the same day. This saves time by eliminating an additional office visit.

From Check-in to departure, everyone was welcoming, friendly and thorough in their job. Dr. Long, the nurse anesthetist, and all the nurses I interacted with were great and caring. The entire procedure from check in to departure was less than 2 hours. Highly recommend Dr. Long and her staff!
